DOCUMENTS REQUIRED FOR THE APPLICATION

Dear Students

You need to prepare the documents required for the hospital applications of the applied courses by logging in to the online student portal (https://auportal.bakircay.edu.tr) with your corporate account information with @bakircay.edu.tr extension. The fields in the portal will be filled in completely and the printout of the application form will be signed by the institution to be interned. After the signature of the institution to be interned is completed, the documents must be scanned and sent to the application approval via the portal before the deadline. The document initialed by the commission members will be delivered to the institution to be interned.

Documents Required for Practical Course within the Scope of Applied Course

Photocopy of Occupational Health and Safety Certificate (16 hours, approved by universities)

2. ‘Status Report / Internship Report’ to be obtained from the family physician, vaccination card (Hepatitis markers)

Documents Required for Volunteer Summer Internship Application

1.Intership Application Form Form (Once the internship application form has been completed and signed digitally, it will be uploaded to the portal)

2. Certificate of Prospect (can be obtained from e-government with QR code)

3. Photo

4. Population Registration Sample (can be obtained from e-government with QR code)

5. Internship Agreement (Not a mandatory document. To be filled in if requested by the organisation to be interned)
