DOCUMENTS REQUIRED FOR IMPLEMENTATION
DOCUMENTS REQUIRED TO BEGIN THE CLINICAL INTERNSHIP
Students who are about to begin their clinical internship must submit all of the documents listed below in order to start the internship.
Required documents:
1. Student ID
2. Copy of ID
3. Chest X-ray (For first-year students)
4. Copy of Occupational Health and Safety Certificate (16-hour course, university-approved document, obtained within the last year)
5. ELISA Test (MMR, Hepatitis A, Hepatitis B, etc.)
6. Employee Confidentiality Agreement (Only for classes to be conducted at the City Hospital; will be communicated to the relevant classes by the Departmental Practice Committee.)
Note: The medical report must be submitted to the instructors of the relevant course. The health report must include an interpretation of the chest X-ray results and the hand-delivery of those results, as well as confirmation that ELISA tests have been performed and interpreted, along with the hand-delivery of the test results. The health report must contain the phrase “reports the condition.”
